NPBA Search Reports

To submit a Search Report for the first time, download the Search Report form. Instructions are included within the form. You need an e-mail address to send the form by e-mail.

How to submit a search report.

Download the Search Report form, if not already downloaded.
Fill in the form using the free, downloadable Adobe Reader.
Save the filled-in form on your computer for your records. You can also print the form.
Submit the Search Report. Submitting requires an e-mail application with Internet access.
You will receive a confirmation e-mail that the report was submitted.
Look for your search report in Nose News, if the case is not pending.

For additional search reports, save the Search Report form as a new file (Save As…).
